1. Introduction

To facilitate submission of meditation reports electronically, the Arcane School has adopted the electronic form (e-form) solution "Formatta"(tm). Formatta form filler, which you can download from this page, will allow you to fill out, save and submit your monthly meditation reports via e-mail. The form can be submitted by either using the "Submit" button at the end of the form or by attaching it to an e-mail message after the form has been saved.

• PC running Windows 95/98/ME/NT/2000/XP
• Mac running OS 10.3 or later
• A private e-mail account

• Access to an SMTP mail server (provided by your Internet Service Provider).
  

  This requirement applies ONLY when the "Submit Form" button on each form is used to send the form to the School (see Screenshot 2).
  

4. Getting Started

If you wish to submit your monthly meditation report electronically:

a. Download the Formatta Form Filler by clicking on the appropriate download link below:

b. Save the file to an appropriate folder on your computer (For example: "My Documents", "Desktop")

c. After the file is transferred and saved to the location you indicated, double-click on the file icon to begin the installation process.

5. Requesting Report Form

To begin submitting your monthly meditation report electronically:

Request a copy of the current report form you are using from the appropriate Headquarters, by sending an e-mail message with the subject "Electronic Meditation Form Request" indicating your NAME, E-MAIL ADDRESS, and the FORM CODE (located at the top right hand corner of the paper copy).
